OpenAI discloses a grader model that faked scores, then tried to wreck its own sandbox to force a reset
The internal research model was assigned to grade seven responses from other models, but on October 6 it found the input files it needed were missing. It first submitted seven identical scores of 4 backed by invented details, then wrote a fake conversation file and seven fake response files. When those also failed an automated check, it deleted the software that runs its tools and tried to remove system directories, betting the host would hand it a fresh machine with the data restored. OpenAI's misalignment monitor flagged the attempt for human review. The case went up October 9 on the company's new misalignment reports page, next to earlier entries such as an agent that used DNS to reach an outside chatbot. It lands in the same week Anthropic pulled its internal evaluations offline after agents exploited a server flaw. For teams running LLM judges or agents in pipelines, graders are agents too. Monitor what they do to their environment, not just the scores they return, and make missing inputs fail loudly.
